    List all medications you are taking (include prescription, over-the-counter, herbal or natural products, vitamins and drugs you take as needed, such as Tylenol). Cross off any medications your doctor has told you to stop taking. Carry this card with you so the information is always available in case of an emergency.

    As new medications are prescribed to you, have your physician and pharmacist take a look at your card so they can inform you of any possible side effects or warnings against mixing the items on your list. Some medications can have adverse effects when mixed with certain herbs, drugs or other products.

    Take your medication card with you to see your doctor, go to the ER or have a test or procedure done. Update the card whenever you change a medication or start a new one, receive a vaccination or develop a new allergy.
