Use our Personal Medication Card to help keep track of your medications easily.

List all medications you are taking (include prescription, over-the-counter, herbal or natural products, vitamins and drugs you take as needed, such as Tylenol). Cross off any medications your doctor has told you to stop taking. Carry this card with you so the information is always available in case of an emergency.

As new medications are prescribed to you, have your physician and pharmacist take a look at your card so they can inform you of any possible side effects or warnings against mixing the items on your list. Some medications can have adverse effects when mixed with certain herbs, drugs or other products.

Take your medication card with you to see your doctor, go to the ER or have a test or procedure done. Update the card whenever you change a medication or start a new one, receive a vaccination or develop a new allergy. It is a simple way to organize your medical history and a quick reference when you are in need of specifics such as dates and phone numbers. Remember, you are a vital part of our team in maintaining your optimal health. We want you to be informed and educated on all the treatments we are providing so that you will be aware when a possible complication occurs. You are your own best advocate - never be afraid to ask questions or voice concerns.
